The role serves as the central scheduling and operational coordinator for a single interdisciplinary care team, ensuring that patients receive the appropriate number and type of in-person encounters required under the Your Health proactive, value-based care model. Under the supervision of the Medical Director and clinical department leadership, the coordinator is responsible for coordinating the Skilled Care (Swing Bed) program within the hospital, which includes screening patients for placement, completion of admission assessment and documentation, completion, documentation, and submission of Minimum Data Set (MDS) and daily physical assessments, providing patient and family education, assisting with admission/discharge of patients into the program and developing and maintaining policies consistent with CIHQ and CMS regulations to support the Swing Bed program. The coordinator facilitates evidence-based care, improves systems processes, supports care coordination, facilitates outcomes management, establishes an evolving risk assessment, supports the patient experience, strives for ongoing quality improvement and serves as a role model for inter-professional communication.

• Collaborates closely with, providing oversight as needed to, the Clinical Manager/Charge RN acting as nurse manager, the Medical Director, and the physicians regarding the direct patient care responsibilities within the facility to ensure the provision of outstanding quality of patient care, as defined by the FMS quality goals, and compliance with the pertinent company policies and procedures.
• Demonstrated leadership competencies and management skills for the position, including excellent communication, customer service, continuous quality improvement, relationship development, results orientation, team building, motivating employees, performance management and decision making.

This position involves proficient chart preparation, communication, scheduling of patient visits, appropriate utilization and logging of chronic care management activities, and provider and patient support.
