div>Key Performance Indicators – The Clinical Care Coordinator is responsible to understand performance measures within the Rehabilitation Hospital, and to collaborate with staff to identify opportunities for improvement and potential action plans. When process, policy, or procedure changes are developed by the Program Director or Nurse Manager, the Clinical Care Coordinator is responsible for directing staff to the information, assist with implementation of the changes, working with staff to complete competencies as needed, and providing feedback to the Nurse Manager. This includes working with all staff to assure that nursing care is delivered consistently according to established policies, procedures, and protocols including activities such as creating equitable staffing assignments, mentoring new staff, ensuring nursing presence at and preparedness for team conferences, communication to and follow up with physicians, safety monitors, and timely follow up on occurrence reports as well as customer service recovery as needed.
