The Psychiatric RN Case Manager provides direct nursing interventions, psychiatric triage, medication and disease process education, care coordination, and collaboration with psychiatric providers, medical providers, behavioral health clinicians, community agencies, and support systems. Collaborate with psychiatric providers, primary care providers, therapists, recovery coaches, care coordinators, schools, DCF, DMH/DDS, detox facilities, hospitals, correctional facilities, community agencies.
