Job Responsibilities: · Provide person centered direct care to residents in accordance with the LNA plan of care including but not limited to, bathing, dressing, positioning, toileting, transferring, incontinence care, hair, nail, and mouth care. · Observes resident for pain, intake and output, skin breakdown, changes in mental status, functional status and overall status and reports changes promptly to the nurse.
