Overview: Under the direction of the Community and Social Services Director, the Community Health Representative performs work within the requirements of the Diabetes Grant and Prop 63 for Native Americans, the ICWA program, and the Title V Self-Governance Compact to fulfill community health objectives in those grants. Provides administrative support to the Director of Community and Social Services, including in the programmatic areas of ICWA, food delivery, home visitation, and elder programming.
