<div><strong>The Care Team</strong><br/>The care you deserve, the support you need.</div><div><br/></div><div>The Care Team Hospice is seeking a driven, relationship-focused <strong>Community Care Liaison (CCL)</strong> to grow our presence in the Cuyahoga Falls area. This is a high-impact role for someone who thrives on building connections, developing referral partnerships, and making a difference in the lives of patients and families.</div><div><br/>As a Community Care Liaison, you will be the face of our hospice services in the community. You’ll build and strengthen relationships with physicians, hospitals, and other referral sources to increase awareness, trust, and utilization of our services.</div><div><br/>Your success will be measured by your ability to grow census through strong partnerships, consistent outreach, and a deep understanding of hospice care. We’re looking for professionals who share that purpose and want to make a lasting impact.</div><div><br/><strong>We offer a supportive, growth-focused environment along with a comprehensive benefits package, including:</strong></div><ul><li>Medical, Dental, and Vision Insurance, plus Wellness Programs.</li><li>Career Path Program with opportunities for advancement and increased compensation.</li><li>Generous Paid Time Off and Paid Holidays.</li><li>Tuition Reimbursement and Certification Assistance.</li><li>Mileage Reimbursement.</li><li>401(k) with Company Match.</li><li>Additional Voluntary Coverage .</li></ul><div><br/></div><div><strong>Responsibilities:</strong></div><ul><li>Build and expand relationships with physicians, healthcare partners, and key decision-makers to drive referral volume and market growth.</li><li>Serve as a dedicated advocate for hospice by educating the community and sharing the value of comfort-focused, patient-centered care.</li><li>Achieve and exceed census and admission targets by positioning our hospice as a preferred provider within the community.</li><li>Engage regularly with healthcare providers and referral sources to create strong, collaborative partnerships focused on patient well-being.</li><li>Lead hospice discussions with patients and families, clearly communicating services and guiding the consent process with professionalism and empathy.</li></ul><div><strong><br/>Education:</strong>                       <strong>       </strong></div><ul><li>Bachelor’s degree or equivalent.</li></ul><div><strong><br/>Experience:</strong></div><ul><li>1-2 years healthcare specific outside sales experience preferred.</li><li>1-2 years outside sales experience preferred or considerable transferrable experience.</li></ul><div><br/></div><div><strong>Travel: </strong>Ability to perform regular travel within assigned geography and extended geography for training and meetings.<br/><br/></div><div><strong>Transportation:</strong> Must have reliable transportation.
