Duties include: • Completion of CEC certification to determine eligibility and enroll patients in health plans • Analyze screening tool results and share with Care Team • Link patients to services within BACH and the broader community • Participate in staff team meetings, case conferences and training • Collect data and provides documentation in client files as required by agency and funders • Participate in obtaining deliverables required for various grants, such as, but not limited to, eligibility assessment, socioeconomic needs assessment, and medical disparities • Assist families with immediate crisis intervention, advocacy, problem-solving and other appropriate interventions, as needed • Utilize de-escalation techniques in situations as appropriate • Utilize program evaluation tools (i.e., pre/ post-tests, satisfaction surveys, etc.), analyzing and reporting on outcomes • Provide resources for various health topics, such as nutrition, physical activity, hygiene, sexual health, and substance abuse • Maintain accurate and confidential records of interactions with students, families and other stakeholders.
