You are part of a muti nursing team, with 24/7 on site nursing coverage. We are currently seeking Evening (part time and full time) and Weekend Positions! LPN Position Specifications:<ul><li>This position will ensure that the health and safety of all residents are met.</li><li>Lead and direct your care team along with actively performing cares of each resident as needed.</li><li>The LPN will collaborate with residents, families, physicians and Moments staff to create a person-centered plan of care that enhances the life and care of the resident.</li><li>Must have innate passion for serving seniors and those living with dementia.</li><li>Must possess the skills to communicate effectively with residents, family members, physicians, all vendors and RN Supervisors.</li><li>Must be able to prioritize and organize work effectively and efficiently.</li><li>Commitment to a compassionate care philosophy and whole person approach.</li><li>Excellent judgment and decision-making abilities and problem-solving skills.</li><li>Experience with EHR/EMR (electronic health/medical records), ideally Point Click Care.</li><li>Support the orientation and education requirements of staff.</li><li>Ability to work with a team to develop person-directed care and outstanding sensory and cognitive programming, in a culture of compassion, respect and dignity.</li></ul>Other Requirements:Must be licensed with the State of Minnesota as an LPN and must maintain current and unencumbered licensure.'<ul><li>Must complete a criminal background check from Department of Human Services and may not be on the OIG exclusions list.</li><li>Must be screened for TB.</li></ul>The job description is not intended to be all-inclusive. This level includes a team huddle room for pre-shift hand off, men’s and women’s locker rooms model after high end local fitness center locker areas, a staff breakroom which will make you want to hang out after and before your shift including, foosball, air hockey, Ms. Pacman, reclining couch and love seat and 65” television as well as large kitchen and counter area. The Moments is a ninety-two (92) suite memory care community located in Lakeville. Designed to provide the highest level of compassion and clinical care possible. Our community takes the position of our RN supervisor as an important role in ensuring the continuation of our high quality clinical care through the evening hours.  This position is one of a supervisory role but most important as an educator to our LPN's and CNA's.  Tasked with continual training and educating you get to use your RN license to its fullest purpose.  Our goal with this position is to provide positive reinforcement of the wonderful care we currently provide.  We at The Moments feel that great care of residents and the continuation of our philosophy starts with the great care of our team. A level of calm compassion and thinking on your feet are a must.RN Supervisor Specifications<ul><li>Monitor resident charts and nursing notes</li><li>Fill medication for medication administration by LPN’s & CAN and checks new medications from the pharmacy.
