This individual will be responsible for performing in-home client assessments, creating individualized Service Plan/Care Plan, orienting and monitoring aides providing services to our clients and conducting supervisory visits in the client’s place of residence. Observes, records, and documents signs and symptoms and reports to the physician, Administrator, or Care Coordinator reactions to non-medical treatments, including changes in the Client's physical or emotional condition.
