This role actively manages patient care by coordinating with specialists and their facilities, guiding patients and members through the healthcare process, and demonstrating in-depth knowledge of self-insured employer health insurance, including copays, co-insurance, and related benefits. You will play a key role in facilitating the patient and member experience, ensuring that primary care is delivered as the patient-centered home, following evidence-based medicine guidelines.
