Prepares or supervises preparation and accurate dispensing of medication to inpatients and outpatients, including unit dose dispensing, automated dispensing machines (Pyxis), IV admixture, chemotherapy, TPN, investigational drugs, other sterile products, and extemporaneous preparations, in accordance with hospital policy. Performs duties including but not limited to medication counseling, therapeutic and formulary substitutions, medication reconciliation, pharmacokinetic monitoring, and renal dose monitoring.
