We're seeking an experienced Registered Nurse who can confidently provide field visits, support admissions, manage urgent patient needs, and step into the Case Manager role when census demands or staffing coverage requires it. Unlike a traditional hospital or clinic-based nursing position, this role requires daily travel between patient locations, independent clinical decision-making, and the ability to build meaningful relationships with patients and families in their own environments.
