It includes assessing, planning, implementing, coordinating, and evaluating health and community related services to meet a patient’s physical, psychosocial, vocational, and health needs in a cost effective, outcomes-oriented and timely manner. As department needs change, the services may be performed at any level within the Network’s continuum of services and/or sites.JOB DUTIES AND RESPONSIBILITIES:<ul><li>Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.</li><li>Performs admission review on all inpatients. Attends daily patient rounds and shares professional knowledge, making appropriate recommendations regarding clinical and programmatic needs of patients as it relates to professional knowledge base and existing treatment criteria.</li><li>Participates in individual patient care meetings with multidisciplinary team members and the patient/family to evaluate progress and to identify and resolve problems that may interfere with a positive patient outcome.</li><li>Facilitate individual sessions with patient & family, and any other significant others according to therapeutic needs and program practices</li><li>Assesses discharge planning needs to ensure a safe, timely and efficient discharge. Identifies variances during the patient’s stay in order to evaluate and improve processes that affect the efficiency and quality of patient care.</li><li>Arranges for community services (including short and long-term placement) prior to discharge to meet patient’s needs with recognition and documentation of patient choice of service providers. Demonstrates effective use of these resources within established reimbursement guidelines.</li><li>Provides patient education and advocacy as appropriate.</li><li>Maintains knowledge and awareness of mental health laws regarding patients’ rights and voluntary/involuntary commitment proceedings, coordinating such proceedings with necessary personnel.</li><li>Maintains appropriate, timely, and effective communication with patients, families, community agencies, physicians, nursing staff, department managers, ancillary departments, and the Manager regarding variances, aspects of patient care management.</li><li>Facilitates insurance authorization, pre-certification, and concurrent reviews for assigned patients.</li><li>Maintains clinical records of all patient contact and clinical reviews, according to hospital policy and other regulatory guidelines (i.e., State, JCAHO). Documentation is completed in a professional and timely manner and includes interventions, recommendations, and referrals.</li></ul>PHYSICAL AND SENSORY REQUIREMENTS:Sitting for up to 5 hours per day, 2 hours at a time. Standing for up to 1 hour per day, 1 hour at a time. Walking for up to 2 hours per day, 20 minutes at a time. Occasionally uses fingers to turn pages. Frequently uses hands to write, type, and hold charts. Rarely uses the hands or fingers for firm grasping or twisting/turning. Frequently uses upper extremities to lift and carry up to 10 pounds.  Frequently stoops, occasionally squats, rarely reaches above shoulder level. Hearing as it relates to normal conversation in person or on telephone. Seeing as it relates to general vision, near vision, far vision, color vision, and peripheral vision.EDUCATION:Registered Professional Nurse with a current license to practice in the state of Pennsylvania. Bachelor's Degree in Nursing requiredOR Licensed Social Worker, MSW, Licensed Professional Counselor (LPC) or Mental Health Professional with a master’s degree in a human services related field required.TRAINING AND EXPERIENCE:At least 2 to 5 years of clinical experience providing care to patients with mental health illness.  Previous experience in discharge planning and case management models preferred. Crisis Prevention Institute (CPI) training required within 90 days of hire. The Organizational Development (OD) Training Specialist will learn, plan, develop, implement and evaluate development sessions for teams across the Network. The OD Training Specialist can expect to facilitate four to five in-person sessions per week at any Network location. The intent of this job description is to provide a summary of the major duties and responsibilities of this position and shall not be considered a detailed description of all the work requirements that may be inherent in the position.JOB DUTIES AND RESPONSIBILITIES:<ul><li>Conducts in-person soft skill related training for groups and teams across the Network; demonstrates flexibility to travel to any Network location to facilitate in-person training events.</li><li>Confident and charismatic facilitation style with ability to adapt to audiences at various levels.</li><li>Demonstrates competency in core department curriculum; identifies and recommends adjustments, updates and enhancements to curriculum based upon direct observation, research and industry trends.</li><li>Researches, creates and adapts original and visually compelling training materials as directed; proficient in creative visual tools like PowerPoint and Canva.</li><li>Primary owner of regularly scheduled virtual sessions for general audiences.</li><li>Assists in administration of assessments and the collection of results and materials for scheduled events; monitors incoming departmental requests via ticket and email.</li><li>Suggests and creates relevant content for bimonthly department newsletter.</li><li>Collaborates with OD consultants regarding core curriculum updates and enhancements; shadows consultant retreats and events for development.</li><li>Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.</li><li>Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.</li><li>Demonstrates/models the Network’s Service Excellence Standards of Performance in interactions with all customers (internal and external).</li><li>Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety. Experience in a formal classroom setting or equivalent professional training environment required.
