Management of inpatient consults, post-operative, and discharge planning needs including ongoing assessment and adjustment to plans in collaboration with Orthopedic team members; collaborative care of inpatients includes examinations and chart review, appropriate ordering and monitoring of diagnostic services (including labs, imaging, specialized studies), ancillary services (including social work, child life, rehab therapies), treatments (including medication reconciliation, immobilization, orthotics, wound vacs, drains, casting, pain management strategies, consultations) and patient and family education. The advanced practice provider (APRN, CNP or PA-C) provides clinical care to patients in a family-centered model with complex acute, critical and chronic health care conditions, advises patients on health maintenance and disease prevention, and/or provides skilled assistance in navigation of the health care system as appropriate to the developmental, cultural, emotional, and medical, surgical, or rehabilitative needs of the patient and their families.
