Support and guide patients and families during evenings, nights, weekends, and crisis situations, ensuring they feel informed, reassured, and cared for throughout the hospice journey. Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
