What You'll Do: Proficient to work within scope of nursing certification that includes, but is not limited to lactation consulting, advanced wound ostomy care, care/case management certification, diabetes educator or asthma educator certification. In this role, you'll help patients navigate their healthcare journey through clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience.
