Over time, these care partner relationships lead to knowledge of each resident's "Life Story" which further helps the Person-Centered Care team to develop deeper relationships and personalized/individualized approaches to care partnering for each resident. Services, the LPN directs and manages resident care including resident assessments, administration of medications, care, planning, supervision of staff, licensure compliance, and being a liaison for residents, family and staff.
