Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care. Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.</p><h4>Qualifications</h4><ul><li>Must possess a high school diploma or equivalent. </li><li>Must either 1) be a licensed practical or vocational nurse in the state in which they currently practice, with at least one year of clinical experience in a healthcare setting; or 2) have at least one year of home health, hospice, or pediatric experience within the last 24 months, and have a demonstrated understanding of staffing and scheduling requirements related to home care services. </li><li>Must have basic demonstrated technology skills, including operation of a mobile device.</li></ul><p>Education and experience, preferred</p><ul><li>Previous experience in home health, hospice, or pediatrics is preferred.</li></ul><p>Requirements*</p><ul><li>Must possess a valid state driver license</li><li>Must maintain automobile liability insurance as required by law</li><li>Must maintain dependable transportation in good working condition</li><li>Must be able to safely drive an automobile in all types of weather conditions<em>* For employees located in Oregon, requirements related to driving are not applicable unless employee has a clinical license</em></li></ul><h4>Additional Information</h4><p><em>Enhabit Home Health & Hospice is an equal opportunity employer.
