GENERAL RESPONSIBILITIES:  Assistant Coaches are responsible for assisting in recruiting, coaching, overseeing practices and games, and any other duties that may be required of a coach or assigned by the Head Coach and/or Athletic Director. DESCRIPTION OF ESSENTIAL DUTIES:  <ol><li>Fulfill head coaching in the absence of the Head Coach.</li><li>Assist in researching and recruiting prospective student-athletes for the athletic program.</li><li>Assist Head Coach in instructing all members of the team of all policies, procedures, and training rules applicable to the team.</li><li>Assist in oversight of compliance of all policies, procedures, and training rules applicable to the team.</li><li>Assume responsibility for the conduct of the student-athletes and other involved students at all times.</li><li>Work with the Head Coach in planning, scheduling, and developing a season schedule.</li><li>Assume the responsibilities for the development of a competitive athletic program within the entire program.</li><li>Drives/transports team members to away events; maintains driver license and satisfactory Motor Vehicle Record.</li><li>Instruct the student-athletes on the fundamentals, proper training methods, strategies, safety precautions, and health measures pertaining to the sport.</li><li>Assist in conducting and attending regularly scheduled practices and games.</li><li>Assist with the recording and tabulating of game statistics.</li><li>Assists in Managing expenditures allotted for meals, lodging, entry fees or any other necessary expenses.</li><li>Assist in fundraising efforts.</li><li>Work with the Head Coach in researching and selecting equipment and supplies for the sport.</li><li>Responsible for the recording, tabulating and reporting of game and season statistics with Black Hawk College, Region IV, and NJCAA website.</li><li>Responsible for public relations (newspaper, television, and other local and national media).</li><li>In the absence of the Head Coach, attend and participate as decision-maker in all meetings required by the Region IV, Arrowhead Conference and/or the Athletic Director.</li><li>Assists in the planning of the Regional or District Tournament when the Quad City campus is hosting the event.</li><li>Assist and participate in the budget administration of the athletic program.</li><li>Monitor the academic progress of each student-athlete in this course and in other courses in conjunction with the Head Coach.</li><li>Assist in teaching the student-athletes the concepts of time management and appropriate balancing of coursework schedules.</li><li>Develop and teach the student-athletes the concepts of teamwork, morale, sportsmanship, courtesy, fair play, academic excellence, and strict adherence to rules of training and conduct.</li><li>Responsible for planning and conducting award activities.</li><li>Responsible for submitting Annual Report to the Director of Athletics including season records, individual statistics, athletic award winners, goals for the next season and his/her inventory of equipment.</li><li>Maintains a satisfactory Motor Vehicle Record.</li></ol>  DESCRIPTION OF OTHER DUTIES: <ol><li>Performs other duties as assigned by Head Coach or by Athletic Director.</li></ol>  OVERALL ACOUNTIBILITIES:<ol><li>Demonstrates BHC core values.</li><li>Accuracy and Efficiency of work.</li><li>Ability to work cooperatively with co-workers.</li><li>Ability to communicate in pleasant manner with employees, student, vendors, and general public.</li><li>Ability to successfully perform above job duties.</li></ol>  PRINCIPAL WORKING RELATIONSHIPS: <ol><li>Works directly with the Head Coach, Director of Athletics, and Office Assistant.</li><li>Works directly with the Athletic Department Staff.</li><li>Works indirectly with the BHC Faculty and Staff.</li></ol> Education & Experience(To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The LTSS Care Coordinator _(Care Coach 1)_ employs a variety of strategies, approaches, and techniques to manage a member's health issues. The LTSS Care Coordinator _(Care Coach 1_ ) understands own work area professional concepts/standards, regulations, strategies, and operating standards, and makes decisions regarding own work approach/priorities and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation. Key Responsibilities + Visits Medicaid members in their homes, Assisted Living Facilities (ALFs), and/or Long-Term Care Facilities and other care settings – 75-90% local travel _(see Additional Information section_ ). + Ensure members are receiving services in the least restrictive setting to achieve and/or maintain optimal well-being by assessing their care needs. + Identifies and resolves barriers that hinder effective care. + Plans and implement interventions to meet care needs. + Coordinates services, monitors, and evaluates the case management plan against the member's personal goals. + Ensures the member progresses towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. + Guides members/families towards resources appropriate for their care. Services are driven by facilitating interactions with other payer sources, providers, interdisciplinary teams, and others involved in the member’s care as appropriate and required by our comprehensive contract. Use your skills to make an impact Required Qualifications The Care Coordinator _(Care Coach 1)_ must meet one (1) of the following requirements: + Bachelor’s degree in social sciences, social work, human services, or a related field. + An active, unrestricted Licensed Practical Nurse (LPN) in the state of Illinois with one (1) year of experience in conducting comprehensive assessments and provision of formal services to elderly individuals. The Care Coordinator _(Care Coach 1)_ must meet ALL of the following requirements: + Applicants must reside in Illinois Region 2 counties or within a 10-mile radius of the region in a bordering county or neighboring state. Eligible counties include: _Boone, Carroll, DeKalb, DuPage, Grundy, Jo Daviess, Kane, Kankakee, Kendall, Lake, Lee, McHenry, Ogle, Stephenson, Whiteside, Will, and Winnebago._ + One (1) or more years of experience in health care and/or case management. + One (1) or more years of experience working with Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services (HCBS), and/or managed care organizations. + Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook. + Ability to use a variety of electronic information applications/software programs including electronic medical records. + Exceptional communication and interpersonal skills with the ability to build rapport with internal and external customers and stakeholders. + Ability to travel in the region to meet face to face with members and/or their families, community partners and care teams. + This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program.
