Join our team and be rewarded with competitive compensation and flexible scheduling while making a difference in the community!</p> Responsibilities: <p style="margin-bottom:0in"><strong>About the Role</strong></p><p style="margin-bottom:0in"> </p><p style="margin-bottom:0in">During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers: </p><ul><li style="margin-bottom:0in">Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li><li style="margin-bottom:0in">Administer and order point of care and lab-based testing as needed</li><li style="margin-bottom:0in">Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate</li><li style="margin-bottom:0in">Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary</li><li style="margin-bottom:0in">Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities</li></ul><p style="margin-bottom:0in"><strong>Benefits of Working at Matrix</strong></p><ul><li style="margin-bottom:0in"><strong>Flexibility</strong> – Visits are scheduled based on YOUR availability. We conduct assessments Sunday – Saturday from 8 am – 7 pm. You can work during the day, evening, and/or weekend based on your schedule. </li><li style="margin-bottom:0in"><strong>Competitive Pay</strong> – Earn income based on the number of assessments you complete. Increase your earning potential by offering greater availability and traveling to see members outside your home location. Those completing a monthly average of 20 – 100 visits <strong>earn $1,800 – $11,500</strong> per month. This is based on the per assessment completion rate range of <strong>$90 – $115</strong> depending on your state of residence.  </li><li style="margin-bottom:0in"><strong>Benefits </strong>– Including mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits. </li><li style="margin-bottom:0in"><strong>Education and Support</strong> – We invest in YOU!
