You’ll partner closely with patients, families, physicians, advanced practice providers, and other members of the multidisciplinary transplant team to coordinate care, provide education, triage patient needs, and help patients successfully navigate life after transplant. If Bachelors Degree: 3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg or If Associates Degree: 5+ years of acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg.
