The RN collaborates closely with patients, families, and the interdisciplinary care team—including therapists, social workers, and physicians—to coordinate care, monitor progress, and provide education and support. This role involves conducting comprehensive patient assessments, developing and implementing individualized care plans, and documenting OASIS data accurately to support quality patient outcomes and reimbursement requirements.
