Under the direction of clinical leadership, the Nurse Care Coordinator lead ensures seamless care delivery for children with chronic conditions by coordinating clinical appointments, providing patient and family education, managing medication authorizations, and serving as a primary point of contact between families, providers, and pharmacies. The Nurse Care Coordinator Manager (RN) is an integral member of Clarity Pediatrics interdisciplinary care team, collaborating with physicians, advanced practice providers, psychologists, and therapists.
