Working collaboratively with Care Management, Pharmacy, Clinical Operations, Quality, Population Health, Information Services, and operational leadership, the analyst gathers business requirements, translates operational needs into technical solutions, and implements workflow enhancements that improve care coordination, patient engagement, and clinical performance. The analyst is responsible for designing, configuring, supporting, and optimizing Epic Healthy Planet functionality, including care management workflows, patient registries, quality reporting, decision support tools, outreach programs, wellness initiatives, and pharmacist-driven clinical programs.
