Preferred Qualifications: • Experience in Value-Based Care or Population Health, including Medicare Advantage documentation and Health Risk Assessments • 5+ years' experience as a Family NP, Primary Care NP, or in Internal Medicine, Cardiology, Nephrology, or Endocrinology • Experience managing population health with a focus on reducing hospitalizations • Prior experience with Cerner or similar EHR systems. Clinical Care & Evaluation: The primary responsibility is completing Comprehensive Health Evaluations (CHEs) to assess and manage comorbid conditions while addressing medical, social, emotional, and financial needs.
