In this hybrid role, you will conduct in-person preadmission evaluations and assessments throughout Kitsap Lake and its surrounding areas. As a Clinical Evaluator, you will perform face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals in need. Through these essential evaluations, you’ll play a key role in promoting well-being, enhancing quality of life, and advocating for services that make a lasting difference in the communities we serve. This full-time Clinical Evaluator position offers both flexibility and purpose—combining remote work from your home office with local travel. Acentra Health covers and reimburses all travel-related expenses, including mileage and parking. Additionally, we provide all necessary technology and equipment for your home office setup, including a laptop for use while in the field.  Job Responsibilities<ul dir="auto"><li>As a Clinical Evaluator (PASRR), you will conduct accurate screenings and evaluations, developing informed level-of-care recommendations based on your clinical assessments.</li><li>Complete precise PASRR reviews by assessing and determining the appropriate level of care based on patients’ physical, behavioral, and psychological functioning. </li><li>Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations.</li><li>Ensure compliance with contractual and regulatory standards while developing level of care recommendations.</li><li>Assess and analyze patients' psychological, emotional, behavioral, and physical functioning to inform clinical decisions.</li><li>Provide well-informed level of care recommendations based on clinical evaluations.</li><li>Ensuring compliance with established workflows, including re-evaluations of records based on psychiatric input.</li><li>Collaborate effectively with healthcare professionals, patients, families, and internal stakeholders as needed.</li><li>Strong attention to detail with the ability to efficiently prioritize and execute tasks in a timely manner. </li><li>Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities. .</li></ul>Qualifications:Required Qualifications, Knowledge and Experience<ul><li>Must possess an active, unrestricted license in the State of Washington in a healthcare or related field, such as Licensed Psychologist (LP), Licensed Professional Clinical Counselor (LPCC), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Professional (LMHP), Registered Nurse (RN) and Psychiatric Mental Health Nurse Practitioner (PMHNP). </li><li>Graduation from an accredited college with a relevant degree program and a minimum of 2 years of experience in assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.</li><li>Demonstrated expertise in assessing and analyzing an individual’s psychological, emotional, behavioral, and physical functioning.</li><li>Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements per State and Federal Regulation</li><li>Experience with public mental health and medical programs, including public sector services and supports, with a solid understanding of Level of Care guidelines and related standards of practice. </li><li>Knowledge of standards of practice related to the determination of Level of Care and standards of practice related to mental health, geriatrics, and other disorders commonly necessitating long-term care placement.</li><li>Effective communication skills with the ability to interact effectively with individuals and groups through verbal, written, and electronic formats.</li><li>Detail-oriented, with the ability to prioritize and execute tasks efficiently within established timelines.</li><li>Proficient in using computer-based software and Microsoft Office Suite (Word, Excel, Outlook, Teams, etc).</li><li>Experience diagnosing psychiatric disorders using the latest DSM standards applicable to the state is a plus.</li><li>Familiarity with public mental health systems and medical assistance programs is a plus.</li><li>Prior experience working with the geriatric population is preferred.</li></ul>   Why usWe are a team of experienced and caring leaders, clinicians, pioneering technologists, and industry professionals who come together to redefine expectations for the healthcare industry.
