We're seeking an experienced RN (transplant experience and CCTC certification are highly valued) to manage comprehensive patient evaluations, facilitate waitlist coordination through the national registry, and collaborate with our robust team of surgical providers, nephrologists, pharmacists, dietitians, and social workers using Cerner Powerchart, OTTR, and Microsoft Teams. This position provides comprehensive management and coordination of care for patients requiring kidney, pancreas, liver, heart, lung or Stem Cell Transplantation The level and intensity of care coordination provided is situational and appropriate based on patient need and payor requirements.
