Provide psychosocial assessments and behavioral health support for patients and families• Develop individualized treatment and support plans based on patient needs and goals• Deliver counseling, education, and resource coordination services• Assist patients and families in navigating community resources and support systems• Collaborate with physicians, therapists, case managers, and other members of the treatment team• Advocate for patients throughout their rehabilitation journey• Support emotional adjustment, coping, and successful community reintegration. Master''s degree in Social Work• Current LCSW licensure in Oregon• Strong communication, counseling, and care coordination skills• Experience working with neurological, rehabilitation, behavioral health, or medical populations preferred• Passion for helping individuals achieve meaningful outcomes and improved quality of life.
