The LPN plays an integral role in all services provided by the mobile crisis team, including rapid response to community crises, assessments, crisis stabilization planning, trauma-informed approach, social determinants of health screening and referrals, community engagement, and linking individuals to other services to meet their immediate and long-term needs. Complete a focused nursing evaluation at the site of the encounter, when dispatched with the Mobile Crisis Team, which includes but is not limited to attaining the client's medical history, current medical concerns, medications, and recent hospitalization and share all pertinent information with the Registered Nurse (RN); if there is an escalation or imminent danger during the initial encounter, immediately engage emergent resources, e.g.
