The Licensed Practical Nurse works within the health care team and provides focused community and clinic health services directed towards the prevention of disease, the promotion of health, the reduction of disability, improving the clients self-care, using professional judgment in focused/limited assessments, screening, contributing to care planning, intervention, and provides health information/education to patients.
