<div><p>The <strong>Home Health Coordinator</strong> is responsible for scheduling and coordinating patient visits across disciplines to ensure timely, efficient, and compliant home health service delivery. This role supports clinician productivity, episode management, and patient satisfaction while working closely with clinical teams, patients, and families.</p><p><strong>Location:</strong> On-site / New Milford, CT Office</p><p><strong>Schedule:</strong>  Full-Time / Monday - Friday / 8:30am -4:30pm</p><h2><strong>Key Responsibilities:</strong></h2><h3><strong>Scheduling & Care Coordination</strong></h3><ul><li>Maintain accurate patient schedules, ensuring all ordered visits are appropriately assigned across disciplines</li><li>Schedule all Start of Care (SOC) and Resumption of Care (ROC) visits in the scheduling system</li><li>Assign and maintain a primary case manager for each patient</li><li>Coordinate clinician coverage for absences, including vacations and sick time</li><li>Communicate with patients and families regarding visit schedules and changes</li></ul><h3><strong>Episode Management & Compliance</strong></h3><ul><li>Ensure visits are completed timely to support billing, compliance, and quality outcomes</li><li>Reconcile scheduled visits against physician orders to ensure regulatory adherence</li><li>Monitor recertification timelines and provide weekly reminders to clinical staff</li><li>Collaborate with clinical managers to support effective episode management</li><li>Support scheduling strategies to minimize Low Utilization Payment Adjustments (LUPAs)</li><li>Maintain working knowledge of Medicare guidelines and payment models</li></ul><h3><strong>Productivity & Operational Oversight</strong></h3><ul><li>Monitor clinician schedules to ensure productivity expectations are met</li><li>Run and analyze reports related to visit completion, timeliness, and scheduling accuracy</li><li>Identify scheduling gaps, inefficiencies, or risks and proactively resolve them</li><li>Partner with leadership to ensure balanced coverage across service territories</li></ul><h3><strong>Communication & Collaboration</strong></h3><ul><li>Serve as a liaison between patients, families, clinicians, and leadership</li><li>Communicate scheduling updates and important information to field staff</li><li>Collaborate with interdisciplinary teams to ensure coordinated, patient-centered care</li></ul><h2><strong>Qualifications</strong></h2><ul><li>2+ years of healthcare experience required, including medical terminology.</li><li>Bachelor's degree or licensed or experienced high level functioning Medical Assistant, CNA, Coder, or other Health care personnel experience.</li><li>Minimum one year of community health experience preferred, with a strong knowledge base of home health or hospice and associated regulations.</li><li>Knowledge of Medicare guidelines, episode management, and LUPA implications preferred</li><li>Strong organizational, multitasking, and problem-solving skills</li><li>Proficiency with EHR and scheduling systems</li><li>Exceptional customer service skills including phone manner and ability to build relationships in a fast-paced environment.</li></ul><br/><div><strong>Why RVNAhealth?</strong></div><div>RVNAhealth (formerly known as RVNA, the Ridgefield Visiting Nurse Association) is a twelve-time winner of the Hearst Media Top Workplaces Award. <h2>Job Description</h2><p>Be a part of a world-class academic healthcare system, <strong>UChicago Medicine</strong>, <strong>Advent Health</strong> as a <strong>Clinical Documentation Specialist.</strong>This is a remote, work from home opportunity and you may be based outside of the greater Chicagoland area.</p><p> </p><p>The Clinical Document Specialist (CDS) is responsible for facilitating improvement in the overall quality and completeness of medical record documentation on a concurrent basis.  The CDS promotes modifications to medical record documentation to ensure accurate depiction of the level of clinical services provided to the patient and to completely describe patients’ severity of illness. The CDS obtains appropriate clinical documentation through collaborative communications from having developed positive working relationships with physicians, nursing staff, other patient care providers, and the Health Information Management Department Coders. The CDS is also responsible for providing informal and formal education to all members of the care team regarding clinical documentation. </p><p> </p><p><strong>Essential Job Functions</strong></p><ul><li>Reviews and re-reviews inpatient medical records for identified payer populations (primarily DRG/Case Rate payment methodology ) beginning approximately 48 after admission. Analyzes the medical record regarding the clinical status of patient, current treatment plan and past medical history and seeks clarification of incomplete, unclear, and ambiguous clinical documentation</li><li>Communicates with attending physician(s) verbally and through written methodology to validate observations, and obtain clarification of incomplete, unclear, and ambiguous documentation including clarification of POA</li><li>Demonstrates and applies basic knowledge of documentation requirements and guidelines in accordance with ICD-9 CM Official Guidelines for Coding and Reporting. </li><li>Works closely with HIM coding staff to assure documentation of principal diagnosis and any co-existing comorbidities and complications accurately reflect the patient’s clinical status and level of services provided.
