Notify physician, when necessary, of changes in residents' conditions, weight, medication errors, medication not available, abnormal laboratory/X-Ray results and any other unusual observations. Work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies and ancillary service providers.
