p>In This Role You Will: The Home Health Aide: • Provide personal care and assist with mobility or exercises under RN/therapist supervision • Perform light housekeeping, meal prep, and support medication management • Observe, report, and document patient condition • Maintain safety, infection control, and proper use of equipment • Communicate effectively with patients, families, and the healthcare team • Participate in training, care planning, and other duties as assigned.
Who You Are: Must be 18 years of age High school diploma or GED Home Health Aide or Certified Nursing Aide license in the state of Indiana required Demonstrated ability to read, write, and follow a Plan of Care Good verbal, written and interpersonal communications skills required Licensed driver with automobile insured in accordance with state and/or Agency requirements.
