As applicable to assigned area(s): Provides individual, group, and family therapy, clinical case management, and outreach services in an effort to fulfill the patient’s treatment goals, wellness and recovery; Participates in development of individual and/or group treatment plans utilizing a variety of brief psychotherapeutic, psychosocial and/or psychoeducational modalities. Works with medical, clinical and support staff to arrange, mobilize, utilize, and coordinate a comprehensive range of community resources that effectively meet the identified patient and family needs as applicable to assigned area(s).
