Contact patients to inform of pertinent post-discharge results per callback guidelines and perform necessary follow-up actions which include, but are not limited to, prescribing medications, coordinating outpatient follow-up care, and patient education. The APP will utilize established protocols to determine the need for further evaluation, testing, or treatment and will communicate this information directly to patient and/or other healthcare providers.
