WHAT WE'RE LOOKING FOR:
The Director of Social Services is primarily responsible for the implementation of social care plans and resident assessments to ensure that the medically related emotional and social needs of the residents are met and maintained on an individual basis.  Key Responsibilities<ul><li>Plan, develop and organize the social service programs of the Health Campus.</li><li>Participate in discharge planning, development and implementation of social care plans and resident assessments.</li><li>Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.</li><li>Provide information to residents/families on Medicare/Medicaid and other financial assistance programs available to the residents.</li><li>Participate in IHCC allowed activities such as the development and oversight of a healthcare support plan that includes coordination of medical care and proactive care management of both chronic diseases and complex conditions such as recurring falls, depression and dementia. Assist in transitional care planning from hospital or nursing facility to home/assisted living.</li><li>Coordinate outside support services for residents to meet their care needs at home or in the health campus.</li></ul>Qualifications for Director of Social Services I: Indiana locations<ul><li>Bachelor's or advanced degree in Social Work or a Human Services field, including but not limited to: <ul><li>Special Education</li><li>Rehabilitation Counseling</li><li>Psychology</li></ul></li><li>Three (3) to five (5) years’ experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment</li></ul>MI, KY, OH, WI locations<ul><li>High school diploma or equivalent</li><li>Bachelor’s in Psychology; or</li><li>Licensed practical nurse in Kentucky, Michigan, or Ohio</li><li>Must meet State requirements and regulatory code(s) for licensure or certification</li><li>Three (3) to five (5) years’ experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment</li></ul>Qualifications for Director of Social Services II:<ul><li>Bachelor's degree in social work</li><li>LSW preferred</li><li>Must meet State requirements and regulatory code(s) for licensure or certification</li><li>Three (3) to five (5) years’ experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment</li></ul>Compensation will be determined based on the relevant license or education held, as well as the candidate’s years of experience

The Patient Assistance Program Coordinator helps patients seeking assistance through pharmaceutical medical device or financial patient assistance programs with processing required forms and ensures hospital receipt of replacement medication or devices when necessary. The Patient Assistance Program Coordinator helps patients seeking assistance through pharmaceutical medical device or financial patient assistance programs with processing required forms and ensures hospital receipt of replacement medication or devices when necessary. The SSL will develop collaborative relationships with community partners and patients developing strategies connecting patients with resources to improve their overall health.Responsibilities:<ul><li>Work with multidisciplinary team and providers to create standard care plan on patients. Review care plan and/or behavioral contract with patient as needed.</li><li>Reassess and modify care plans as needed.</li><li>Provide appropriate interventions which demonstrate knowledge of and sensitive toward cultural diversity, health literacy, and educational backgrounds of the population served.
