Our unit patient population consists of advanced heart failure and stroke diagnoses including, advanced Heart Failure (HF), MI, Pre/Post Cardiac Catheterization and/or intervention, Pre/Post Pacer/ICD placement, Stroke patients post TNK after an ICU stay, those not eligible for TNK, ruling out for Stroke/TIA, and medical issues including COPD, Pneumonia, ACS, chest Pain and patients requiring inotropic or vasodilator support to enhance diuresis. As an integral member of the Yale New Haven Health System (YNHHS) healthcare team, the Registered Professional Nurse (RN) upholds the YNHHS mission, vision, values and strategic initiatives to provide the highest level of patient centered care.
Available via cell phone to respond to Triage visit requests, and communicates effectively with the on call team, including Hospice physician to address urgent patient needs, and establish a safe plan of care. Responsible for providing on call patient visits scheduled, or unscheduled, for urgent physical and psycho-social issues during after hours scheduled shift, in coordination with Call Center Triage and On Call Manager.
