Responsibilities:
<ul style="margin-top:0;margin-bottom:0"><li style="font-size:11pt;font-family:'poppins' , sans-serif;color:#000000;font-weight:400">Assess the clinical nutrition needs of residents/patients and develop and document the plan within the electronic medical record.</li><li style="font-size:11pt;font-family:'poppins' , sans-serif;color:#000000;font-weight:400">Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities </li><li style="font-size:11pt;font-family:'poppins' , sans-serif;color:#000000;font-weight:400">Provide nutrition education and counseling to residents/patients, families, and staff.</li><li style="font-size:11pt;font-family:'poppins' , sans-serif;color:#000000;font-weight:400">Collaborate with interdisciplinary teams to determine a plan of care that improves residents'/patients’ overall health and well-being.</li><li style="font-size:11pt;font-family:'poppins' , sans-serif;color:#000000;font-weight:400">Ensure adherence to CMS, State, and Federal regulations and company standards.</li><li style="font-size:11pt;font-family:'poppins' , sans-serif;color:#000000;font-weight:400">Provide coverage in multiple buildings based on company needs, and as desired.</li><li style="font-size:11pt;font-family:'poppins' , sans-serif;color:#000000;font-weight:400">All other duties as assigned.</li></ul>

Qualifications:

Work is governed by local agency protocol, federal regulations and state policies and procedures. Performs related work as required.
