Development, implementation, and management of an individualized, interdisciplinary team plan of care designed to achieve maximum nutrition support. Counseling and education of patient/family in the application of medical nutrition therapy principles when appropriate. Make appropriate referrals to other members of the health care team to optimize outpatient care and follow-up. • Evaluates age-specific developmental and disease-specific nutritional needs of patients.
• Develops and implements individualized nutritional plan of care.
• Evaluates and monitors the effectiveness and outcomes of medical nutrition therapy interventions.
• Identifies patient nutrition education needs and provides nutrition counseling to patients, family, and/or caregivers.
• Participates in the competency assessment process and achieves a passing score of 80 or higher on RD chart audits.
• Adheres to nutrition care guidelines, the nutrition care manual, department, facility guidelines, and/or protocols to provide evidence-based nutrition care.
• Integrates the Academy Code of Ethics in all areas of practice.Qualifications
To be considered for this position, you will need:• A Bachelors Degree in Nutrition, Dietetics, or equivalent required.
• Completion of an ACEND-accredited supervised practice program required.
• One to two years of hospital clinical nutrition experience preferred.
