The Registered Nurse (RN) Case Manager provides comprehensive nursing care to hospice patients by conducting initial and ongoing assessments of the impact of a terminal diagnosis on the patient's physical, functional, psychosocial, and environmental needs, as well as activities of daily living. This role develops, implements, and revises individualized plans of care, provides ongoing education and support to patients and their families, and collaborates closely with the interdisciplinary hospice team to ensure compassionate, high-quality care.
