The RN will be responsible for conducting client assessments, developing and updating individualized care plans, performing supervisory visits, and ensuring caregivers are delivering care in accordance with each client's plan of care. Communicate with clients, families, physicians, and the AbriCare care coordination team regarding changes in client condition, care plan updates, and clinical concerns.
