Diagnose, manage, and treat conditions and diseases of the human eye and visual system. Examine eyes and visual system, diagnose problems or impairments, prescribe corrective lenses, and provide treatment. May prescribe therapeutic drugs to treat specific eye conditions. Thursdays & Fridays (8:00am - 5:00pm)Essential Duties and Major responsibilities:<ul><li>Examine eyes, using observation, instruments and pharmaceutical agents, to determine visual acuity and perception, focus and coordination and to diagnose diseases and other abnormalities such as glaucoma or color blindness.</li><li>Analyze test results and develop a treatment plan.</li><li>Prescribe, supply, fit and adjust eyeglasses, contact lenses and other vision aids.</li><li>Prescribe medications to treat eye diseases if state laws permit.</li><li>Educate and counsel patients on eye care, contact lens care, visual hygiene, lighting arrangements and safety factors.</li><li>Consult with and refer patients to ophthalmologist or other health care practitioner if additional medical treatment is determined necessary.</li><li>Ability to deal with sensitive information in a highly confidential manner.]</li><li>Effective communication skills, both oral and written, with patients, families, visitors, co-workers, health care professionals, funders, and other regulatory agencies.</li><li>Works as part of team.</li><li>Awareness of resources available within the health center to accommodate the needs of patients.</li><li>Demonstrate understanding and commitment of the health center mission.</li><li>Demonstrate understanding and commitment to the established CHC Values and Standards.</li><li>Performs other duties as assigned.</li></ul>Minimum Qualifications:<ul><li>Requires a doctorate in optometry (OD) with a Massachusetts therapeutics level optometry license, and be board certification in optometry.</li><li>Current Massachusetts Controlled Substances required.</li><li>Requires an active commitment to excellence in patient care, a desire to improve wellness of our community, an appreciation for community needs, a culturally competent individual, and someone who is excited about the opportunity to build the eye service.</li><li>Minimum of five years professional experience preferred</li><li>Public Health experience preferred.</li><li>Bilingual a plus.</li><li>Proficient reading, writing, and speaking in English required</li></ul>

In-Home Therapy is structured, consistent, strength-based therapeutic relationships between a licensed clinician and the youth and family for the purpose of treating the youth’s behavioral health needs, including improving the family’s ability to provide effective support for the youth to promote his/her healthy functioning within the family.

She/he is accountable to the Supervising Licensed, Master Level Clinician.  The Master’s level clinician maintains a caseload of 5-8 families, and will liaises with Outpatient Clinician, Care Coordinator and the Family Partners and other team members to achieve program objectives.Duties and Responsibilities:<ul><li>Comprehensive home-based assessment, behavioral health assessment inclusive of the age appropriate version of the Massachusetts CANS that occurs in the youth’s home or another location of the family’s choice.</li><li>Development of a youth and family centered treatment plan in collaboration with the youth, parent/caregiver(s), and with required consents, in consultation with other providers.</li><li>Review and modification of treatment plan as necessary.</li><li>Provides or arranges for transportation for his/her clients to appointments, crisis/respite services, etc. if needed. Driver’s license and auto insurance with adequate coverage must be kept up to date.</li><li>Review/development of a risk management/safety plan in collaboration with the youth and parent/caregiver.</li><li>Provides intensive family therapy that may include working with the entire family, or a subset of the family, to implement focused, structural, strategic, or behavioral techniques, or evidenced-based interventions to enhance problem solving, limit setting, risk management/safety planning, communication, skill building to strengthen the family, and to advance therapeutic goals or improve ineffective patterns of interactions.</li><li>Identification of community resources and development of natural supports for youth and parent/caregiver(s) to support and sustain achievement of the yout’s treatment plan goals and objectives.</li><li>Provides or secures support and crisis/emergency services for the youth/family. This may be done through face-to-face contact, phone contact or by beeper or an on call system.
