Physical TherapistShift: Full time Rate: $34 - $37.73/hr. (depending on experience) Job Summary: We are seeking a skilled Physical Therapist (PT) to join our team in providing exceptional rehabilitation services to residents. As a PT, you will assess, plan, and implement personalized therapy programs to enhance mobility, strength, and overall quality of life for patients in our skilled nursing and rehab facility.Essential Duties:<ul><li>Conduct assessments to determine physical therapy needs and goals.</li><li>Develop and implement individualized treatment plans for residents.</li><li>Provide hands-on therapy to improve strength, balance, and mobility.</li><li>Collaborate with the healthcare team to optimize patient care.</li><li>Document progress and modify treatment plans as needed.</li><li>Educate residents and their families on techniques for improving daily function.</li><li>Adhere to infection control practices and regulatory standards.</li></ul>Required Skills:<ul><li>Current Physical Therapist license in New York State (NYS).</li><li>Strong assessment and therapeutic skills.</li><li>Ability to work effectively in a multidisciplinary team.</li><li>Excellent communication and interpersonal skills.</li><li>Passion for patient care and improving quality of life.</li></ul>About Us: Chautauqua Nursing and Rehab, part of the Absolut Care family, provides top-tier nursing and rehabilitation services.
