Capital Cardiology Associates is looking at posting a part-time APP telehealth remote position to help with TCM visits and triage health calls/tasks. Ideally this would be 8:00am-2:00pm or 9:00am-3:00pm Monday through Friday. Position Overview: We are seeking a highly motivated and clinically skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our cardiology team in a remote, part-time telehealth role. This role is primarily responsible for transitional care management (TCM) and longitudinal remote task management, including working the Health Problem task list, with additional support for urgent symptom triage.  The APP will work collaboratively with triage nurses and cardiologists while maintaining a high degree of autonomy in clinical decision-making.This is an excellent opportunity for both experienced providers and new graduates with a strong background in cardiology, emergency medicine, or critical care.Key Responsibilities include but not limited to:<ul><li>Perform Transitional Care Management (TCM) visits (7- and 14-day as appropriate) and ensure completion of all TCM requirements, including medication reconciliation, review of hospitalization records, coordination of follow-up care and testing. Installation Merchandisers help build Hallmark fixtures, moving card departments, installing new stores, store relocations, and card department remodels.SALARY AND SCHEDULE DETAILS<ul><li>Your starting hourly pay rate will be $22.50 - $24.50depending on your skills and experience.</li><li>We provide paid training, delivered through live, hands-on instruction as well as virtual classroom training.</li><li>Hallmark part time employees are offered voluntary benefits that include medical, dental, vision etc.  </li><li>Eligible Employees receive annual pay increases.</li><li>This is a Part-Time position with a variable schedule during the work week.</li><li>Average weekly hours for this position are between 10-20 hours per week.</li><li>Full availability the week before and after major holidays, which may include weekends. </li><li>Availability to lead and or support season changeovers, extended services, installations, and inventories.</li></ul>YOUR ROLE AND RESPONSIBILITIES WILL INCLUDE  You'll perform supervisory overview and assistance with service work in the Hallmark department of various retail stores such as grocery stores, drug stores, department stores, and mass retailers. The Installation Supervisor position consists of two major components:<ul><li>Supervise and Manage Service:<ul><li>Supervise a team of 2-3 Installation Merchandiser Floaters.</li><li>Document and manage performance with manager feedback and input.</li><li>Handle retailer concerns, retail execution issues and retail merchandiser performance issues by working with store management, field merchandisers and/or your manager.  </li></ul></li><li>Lead Department Remodels and Resets:<ul><li>Prepare, direct and follow-up on minor installation which involve resetting product, building fixtures and merchandising product.</li><li>Lead or participate in major installations when business exceptions require additional support.</li><li>Major installations could involve moving the card department, building fixtures and merchandising product.</li></ul></li></ul><ol></ol><ul></ul>In this role you must be able to motivate both in-person and remote teams, follow through on commitments, and lead by example in applying Hallmark’s best practices. You should excel at managing and leading others through change, delegate tasks when needed, and demonstrate strong problem-solving skills. Being organized, managing time efficiently, and handling administrative duties are also key responsibilities, as well as building and maintaining strong customer relationships.PHYSICAL REQUIREMENTS This is a physically demanding job that requires a high level of energy and a sense of urgency. You will be working on the selling floor as well as in back stockrooms. You must be able to consistently push, pull, lift, and carry cartons, merchandise and display fixtures up to 30 pounds throughout the workday and up to 50 pounds on occasion.
