Job Summary: The Physical Therapist will evaluate all patients/residents admitted to the facility and will recommend, in writing, to the attending physician a rehabilitation program if indicated. Upon receipt of the physicians' orders, the Physical Therapist will be responsible for the interpretation and implementation of service and programs concerning physical therapy.

Physical Therapist RATE: $50.00SIGN ON BONUS: $7,500.00 USDSHIFT: FT DaysJob Summary: The Physical Therapist will evaluate all patients/residents admitted to the facility and will recommend, in writing, to the attending physician a rehabilitation program if indicated. Upon receipt of the physicians' orders, the Physical Therapist will be responsible for the interpretation and implementation of service and programs concerning physical therapy. The physical therapist will, on a regular basis, review and evaluate all patients/residents, be responsible for all documentation of patients/residents' care plans. Attends and actively participates in all staff and committee meetings related to improving patient /resident care and maintaining the physical therapy department. Job Performance Responsibilities: <ul style="list-style-type:disc"><li>Evaluates all patients/residents admitted to the facility within 24 hours</li><li>Recommends in writing to the attending physician a rehabilitation program, if indicated, in their professional's judgement, to include but not be limited to restorative nursing</li><li>Upon receipt of physicians' orders interpret and implement those services and programs related to physical therapy. </li><li>Ensures that orders are carried out in a professional manner, satisfying professionals ethics, patients/residents rights, while maintaining State and Federal Health department codes</li><li>Conducts initial patient/resident evaluation</li><li>Creates short- and long-term goals</li><li>Assesses rehabilitation potential </li><li>Develops and implements a treatment plan which may be delegated to a PTA if applicable</li><li>Engages in professional information communication </li><li>Documents all patient/resident charts and nursing care plans </li><li>Ensures that proper equipment order for home discharged and HEP given as appropriate</li><li>Assigns appropriate therapeutic prescription of minutes while monitoring accordingly</li><li>Competent in long term care, sub-acute, and outpatient therapies </li><li>Provides appropriate patient education </li></ul>Job Qualifications: <ul><li>Bachelor degree of Physical Therapy </li><li>2-3 years' experience in long term care</li><li>Licensed and registered in NYS </li></ul>Why Join us?Weekly PaychecksReferral BonusesPeer Mentorship
