An Internal Medical Group has an opportunity for Advanced Practice Registered Nurse (APRN) or a Physician Assistant (PA) to work Part Time in Skilled Nursing Facilities in the Meriden area.

Salary Range: $110,000.00 to $200,000.00 a year with quarterly bonus opportunity.By adhering to Connecticut State Law, pay ranges are posted. The pay rate will vary based on various factors including but not limited to experience, skills, knowledge of position and comparison to others who are already in this role within the company.  Flu Vaccine Considerations  Proof of annual flu vaccination is required for all employees. Physicians Alliance of Connecticut (PACT, LLC) is a multi-specialty Physician groups. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and accelerate in a pay for performance environment.The Nurse Practitioner or Physician Assistant will travel to Skilled Nursing Facilities contracted with PACT, LLC.Essential Function<ul><li>Provide medical care to a complex patient population in the short term rehab and long term care setting</li><li>Analyze patient’s medical history, medication allergies, physical condition and examination to determine best care for acute and chronic medical conditions</li><li>Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs</li><li>Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate for assessed medical conditions</li><li>Record in medical record the physical findings, formulated plans, and prognosis based on patient’s condition within appropriate time frame</li><li>Coordinate with the nursing facility care planning team in the development of comprehensive care plan</li><li>Communicate with patients and family members regarding  admission, care plan, discharge needs, prognosis/end of life care as needed</li><li>Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards</li></ul> Education and ExperienceAPRN<ul><li>The Advanced Practice Registered Nurse is masters or doctorate prepared</li><li>Certified Nationally as a Nurse Practitioner through AANP or ANCC .</li><li>The Nurse Practitioner must have possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a y) Nurse Practitioner by an approved certifying body.</li><li>DEA, and Ct controlled substance license</li><li>Experience working with elderly patient population</li><li>Experience working with EHR</li></ul>PA<ul><li>Physician Assistant Certification</li><li>State License</li><li>Any years of experience</li><li>Experience working with EHR</li></ul>#APRNCT #NursePractitioner #AdvancedPracticeRegisteredNurse #APRN #PA #PhysicianAssistant
