This position works interdependently with the multidisciplinary team, guiding members of the multidisciplinary team, (e.g., RNs, LVNs, HHAs, PTs, OTs, STs, MSWs, nursing students and newly employed RNs), as appropriate, and supervises patient plans of care including education on Hospice Benefit as appropriate. The NP may perform home visits when physicians/NP level visit is needed for patients in absence of their primary physician of record.
