Team members drive agency vehicles to meet clients in the communities served. Work is done in a remote/hybrid setting and daily travel is involved.Assesses and monitors the client’s on-going psychiatric, medical and social needs. Oversee medication monitoring and administration under team psychiatrist/nurse practitioner. Collaborates with internal and external providers to coordinate psychiatric treatment with medical treatment. Manages pharmaceutical and medical supplies.This position functions as a fully integrated Team Member of the Assertive Community Treatment Team that assists individuals with a chronic mental illness to minimize symptoms and the resulting impairment in daily functioningCompensation: The pay offer will be determined based on relevant experience. Work Schedule: 10 to 20 hours per weekMINIMUM QUALIFICATIONS <ul><li>Bachelors or above in Nursing preferred.</li><li>Two years of experience, preferably in a substance abuse or mental health behavioral health setting along with working knowledge of primary health care issues.</li><li>Nursing license required – RN preferred</li><li>Valid driver’s license/ acceptable driving record</li></ul><ul></ul>Essential Duties and Responsibilities<ul><li>*Plan, implement and evaluate programs of care and therapeutic intervention to meet the needs of an identified caseload of chronically mentally ill individuals.</li><li>*Act as case manager both to clients on own caseload and that of other professionals on the team.</li><li>*Ensure that care given is appropriate and coordinated, incorporating best practice for treatment, risk assessment and multi-agency service coordination.</li><li>*Support individuals and their families during inpatient admission and with their reintegration to the community.</li><li>*Provide psychiatric assessments for individuals to include routine and emergency assessments.</li><li>*Assess physical health needs, make appropriate referrals, and coordinate care as needed.</li><li>*Screening for health risks at time of admission and periodically thereafter.</li><li>*Connect clients who have no primary care providers with a provider.</li><li>*Health Coaching-implement individual and group wellness activities.</li><li>*Providing crisis health intervention to stabilize health of ACT clients when at risk for admission or readmission or ER use.</li><li>*Implementing preventive, as well as remedial, health and wellness plans.</li><li>*Cross-training of other team members in use of health and wellness interventions as individually indicated.</li><li>*Provide management and administration of medication in conjunction with the ACT psychiatrist or nurse practitioner.</li><li>*Work closely with other members of the team and staff from other key agencies.</li><li>Provide a range of therapeutic interventions to individuals and their families.</li><li>*Maintains client clinical record with treatment plan, client progress and termination summary.</li><li>Provide education and training to agency staff regarding health issues.</li><li>Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.</li><li>Cooperates and collaborates with program area staff, volunteers, and other Agency staff.</li><li>Is dependable and punctual regarding scheduling and attendance.</li><li>Abides by all specific program and Agency procedures, policies, and requirements.</li><li>Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.</li><li>*Creates, maintains, and shares as appropriate a dynamic self-care plan. Maintains clinical record.</li><li>*Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agency’s ongoing fundraising and friend-raising efforts.</li><li>*Essential functions of this job are to be performed in a community based program.</li><li>Performs other program related duties as assigned.</li></ul>*DENOTES ESSENTIAL JOB FUNCTIONS
