Provide written reports for each patient, including initial evaluation; progress, summary, and discharge reports; progress notes for each treatment session; physician certification/billing questionnaires and other relevant reports/documentation as needed + Provide counseling, education/training and guidance to patients, patient's families and caregivers as appropriate + Confer with patient, medical practitioners and appropriate others to plan, implement, and assess the treatment program + Act as a consultant and promotes program to educational, medical, social, dental, and other professional groups **REQUIRED POSITION QUALIFICATIONS** A Master's degree in Speech-Language Pathology/Audiology or Communicative Disorders or equivalent degree approved by the American Speech/Language and Hearing Association. **PRIMARY JOB RESPONSIBILITIES** + Review physician's referral and patient's medical record, review diagnosis, and screen/evaluate communication disorders including functional/organic speech, language, voice, fluency cognitive/communication and select swallowing disorders + Develop a treatment plan + Evaluate and monitor patient's progression and make appropriate adjustments to treatment plan to achieve maximum benefit + In individual or group settings, treat and manage patients with communication and/or swallowing disorders such as: + Neurogenic communication disorders: aphasia, apraxia, dysarthria and cognitive/communication deficits.
