p>What you will do: The Licensed Master Social Worker provides advance clinical social work services as a member of an inter-disciplinary team engaging with patients and family members in coping with the patient's hospitalization, illness, diagnosis, treatment and/or life situations including emotional, mental and substance abuse disorders.
In collaboration with the patient and interdisciplinary team, develops, implements, evaluates and revises, as necessary, a plan for hospital and post-hospitalization care, including referrals to other disciplines (psychiatry, addiction medicine, etc.) healthcare and community organizations based on needs assessment.
